IMPROVEMENTS

Quality improvement made for formal documentation of previously closed issues for which customer information request has been made. Previously, no formal tool for reporting problems and their resolution existed outside of the 8D format. An 8D is a rigorous tool and methodology that takes several hours to complete for a single issue, even when done after all information is available. An A3-SCAR form has been developed to be utilized in the case where a previous issue has been found, addressed, and closed and a customer requests a report. This can be completed within a half hour, contains all key information, and is well presentable to the customer. This A3 serves to replace either (1) a myriad of emails, powerpoints, and other messages or (2) a formal 8D with many more hours of effort required.
