Describe the unsafe act or unsafe condition observed as well as the behavior observed, including the immediate countermeasures taken

Located at B 15 the Fan used for Operator cooling has a broken cord at the base of the plug ... wires/ components are fraying out from cord. Risk of electrocution when plugging fan in since its right at the plug where people frequently touch. Also could cause a fire needs to be replaced or repaired

Fan needs to be replaced or cord needs repaired properly.
