| Describe the unsafe act or unsafe condition observed as well as the behavior observed, including the immediate countermeasures taken | The door fell off when opening it to blow out the oil lines. This is very dangerous as it could’ve ripped a hot oil line (360°) and sprayed all over me (Tyler), saif and Steve. |
